Job Summary

To support a growing healthcare practice, the part-time Pennsylvania Licensed Health Coach will provide health coaching and case management remotely, assisting members in managing their health, setting goals, and navigating the healthcare system.

Key responsibilities:

• Engage with members through coaching and intensive case management to promote self-management of health
• Conduct assessments to identify needs and develop care plans while coordinating with healthcare providers
• Provide clinical coaching support for members seeking assistance with health-related inquiries and issues

Required qualifications:

• Current, active, and unrestricted Pennsylvania Registered Nurse licensure
• Three years of experience in clinical, case management, or disease/condition management
• Strong clinical knowledge and basic computer skills
• Access to a personal laptop for remote work
