Job Summary
To support a growing health services team, the full-time contract Pennsylvania Licensed Health Coach will engage with members to provide lifestyle improvement coaching, manage chronic conditions, and develop care plans, all while working remotely from Philadelphia, PA.
Key responsibilities:
- Function as a primary nurse for members, providing support for lifestyle improvement and health-related inquiries
- Conduct assessments to identify member needs and develop tailored care plans
- Maintain ongoing telephonic communication with members to achieve health goals and ensure care coordination
Required qualifications:
- Current, active, and unrestricted Pennsylvania Registered Nurse licensure
- Three years of experience in clinical, case management, or disease/condition management
- Strong clinical knowledge relevant to health coaching
- Basic computer skills
