Pennsylvania Licensed Home Dialysis Nurse

Job Summary

Providing on-call support for patients in the home HHD or PD program, the full-time Pennsylvania Licensed Home Dialysis Nurse will assist with technical issues and patient care concerns while working remotely for three shifts per week, including overnight and weekend coverage.

Key responsibilities

• Manage patient calls, troubleshooting machine issues and addressing patient care concerns
• Follow DaVita protocols and guidelines to ensure quality patient care
• Utilize proprietary software systems for effective communication and patient management

Required qualifications

• Current Pennsylvania RN license with a minimum of 1 year of PD/HHD experience
• Proficiency in using software systems such as Falcon CWOW, Outlook, and Excel
• Ability to work overnight shifts and weekends as required
• Experience as a Kidney Smart Educator is a plus
• Strong critical thinking skills related to patient care and long-term goals
