Job Summary

Pennsylvania Licensed Home Modification Case Manager, this full-time remote position involves collaborating with various stakeholders to facilitate home modifications for participants in Pennsylvania, ensuring their homes are safer and more accessible.

Key Responsibilities
  • Plan and coordinate home modification projects, including evaluating requests and overseeing the process
  • Communicate with therapists, contractors, and other stakeholders to ensure project alignment
  • Manage project budgets and timelines, preparing necessary reports and documentation
Required Qualifications
  • Bachelor's degree preferred or 2 years of relevant experience may be considered in lieu of degree
  • Minimum of two years of general business experience required
  • Competence in MS Office Suite, Adobe, and general PC skills required
  • Experience in project management, home renovations, or general construction is a plus
  • Knowledge of health insurance, Medicaid, and Medicare products is a plus
