Job Summary

A company is looking for a Market Registered Nurse in Pennsylvania.

Key Responsibilities

• Provide compassionate, individualized, and holistic patient care aligned with a value-based care delivery model
• Deliver clinical care and education virtually, focusing on urgent needs and chronic condition management for high-risk patients
• Coordinate care for patients with acute needs and develop individualized action plans to improve clinical outcomes

Required Qualifications

• Bachelor's degree in Nursing and/or commensurate clinical experience
• Active nurse licensure in the state of New York, in good standing
• Recent telephonic triage, urgent care, or ER/ED experience required
• Clinical experience caring for aging adults and adults with complex care needs required
• Advanced training in geriatrics and/or palliative medicine preferred
