Job Summary

A company is looking for a Care Continuum Partner - Medical Assistant.

Key Responsibilities

• Collaborates with the clinical care team for pre-visit planning and patient flow
• Conducts workflows for care gap closures and manages patient outreach for care plan compliance
• Facilitates virtual appointments and provides patients with preparation instructions for tests and procedures

Required Qualifications

• Technical School Diploma or Specialized Diploma as a Medical Assistant
• 2 years of experience interacting with patients and performing core MA functions
• 3 years of experience in a complex health system environment or related field
• Ability to handle multiple tasks simultaneously and maintain strict confidentiality
• Working knowledge of basic computer functionality
