Job Summary
To support a growing Population Health team, a Pennsylvania Licensed Nurse Practitioner will provide telephonic and video-based follow-up care to patients post-hospital discharge, working part-time (24-36 hours/week) in a remote environment.
Key responsibilities
- Manage care needs for the Transitions of Care program through telephonic outreach and scheduled visits
- Conduct medication reconciliation, symptom screening, and coordinate follow-up care with patients and providers
- Collaborate with nursing staff to achieve individual performance targets and contribute to team-level population health goals
Required qualifications
- Certified Nurse Practitioner in Adult, Family, or Geriatrics
- Active Pennsylvania state licensure, with the ability to obtain additional state licenses
- 5+ years of post-graduate clinical experience in Primary Care or Geriatrics
- Experience engaging Medicare and Medicaid populations managing complex, comorbid conditions
- Comfort in a digital-first environment with experience in virtual care delivery
