Job Summary
Seeking a Pennsylvania Licensed Nurse Practitioner, the part-time role will provide telephonic and video-based follow-up care for patients post-hospital discharge, focusing on medication reconciliation, symptom screening, and care coordination across various settings.
Key responsibilities
- Manage care needs for the Transitions of Care program through telephonic outreach and scheduled visits
- Conduct medication reconciliation and symptom screening while ordering necessary follow-up labs and medications
- Collaborate with nursing teams and coordinate care across hospitals, pharmacies, and community providers
Required qualifications
- Active Pennsylvania Nurse Practitioner license
- 5+ years of post-graduate clinical experience in Primary Care or Geriatrics
- Direct experience engaging Medicare and Medicaid populations managing complex conditions
- Motivational interviewing skills for facilitating patient behavioral changes
- Comfort and proficiency in a digital-first care delivery environment
