Pennsylvania Licensed Pediatric RN Case Manager

Job Summary

Pennsylvania Licensed Pediatric RN Case Manager, a full-time and exempt position, is responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating options and services to optimize the health status of patients or members while working from home in Pennsylvania.

Key Responsibilities

• Assess and design individualized care plans in collaboration with patients, families, and healthcare teams
• Monitor and evaluate healthcare needs, treatment plans, and patient progress to ensure quality and efficiency
• Manage utilization metrics and apply evidence-based guidelines to enhance patient-centered care delivery

Required Qualifications, Training, and Education

• Current Pennsylvania RN license
• Minimum of 2 years of relevant RN work experience
• Certified Case Manager (ACMA) required within 2 years of hire
• Bachelor's Degree in Nursing (BSN) preferred
• Strong pediatric nursing experience is highly preferred
