Pennsylvania Licensed Pediatric RN Case Manager

Job Summary

To support a growing pediatric practice, the full-time Pennsylvania Licensed Pediatric RN Case Manager will assess, plan, implement, and coordinate care for patients while working remotely in Pennsylvania.

Key responsibilities

• Assess healthcare, educational, and psychosocial needs of patients and design individualized care plans
• Integrate evidence-based clinical guidelines into treatment plans to promote quality and efficiency in healthcare delivery
• Continuously evaluate clinical metrics and adjust treatment plans as necessary to optimize patient health outcomes

Required qualifications

• Active Pennsylvania RN license
• Minimum of two years of nursing experience
• Certified Case Manager (ACMA) required within two years of hire
• Preferred Bachelor's Degree in Nursing (BSN)
• Strong pediatric nursing experience is highly preferred
