Job Summary

A company is looking for a Physical Therapy Assistant (Care Navigator) to support patient success through remote monitoring and intervention.

Key Responsibilities

• Provide remote monitoring services to support patients in their home-exercise therapy program
• Direct and assist users in therapeutic exercises and functional training through the technology platform
• Maintain necessary records and provide feedback on patient progress to the evaluating clinician

Required Qualifications, Training, and Education

• Graduated from an accredited physical therapy program
• Active PTA license
• 2+ years of experience in musculoskeletal health is a plus
• Tech-savvy with access to a newer model laptop and smartphone
• Experience in behavioral modification or Health Coach Certification is a plus
