Pennsylvania Licensed Practical Nurse

Job Summary

A company is looking for a Remote Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) for a day shift position.

Key Responsibilities

• Manage patient census and close gaps in clinical and non-clinical patient care
• Identify and coordinate community resources for patients to enhance their care
• Provide patient education on chronic condition management and perform medication management

Required Qualifications

• Active Multi-State/Compact License (LPN) or (LVN)
• At least 3 years of direct patient-facing work experience
• Technical aptitude with Microsoft Office Suite
• Ability to work in a sedentary position for extended periods
• Must have a home office or HIPAA-compliant workspace
