Pennsylvania Licensed Precert Care Manager

Job Summary

A company is looking for a Precert Care Manager (Evening Shift).

Key Responsibilities

• Coordinates and maintains daily logs for reporting and analysis of member incidents and quality of care concerns
• Facilitates linkages for members and families between primary care, behavioral health providers, and other agencies to develop service plans
• Completes acute and non-ambulatory precertifications and collaborates with physician advisors for case planning

Required Qualifications

• Current and unrestricted Pennsylvania Licensure in health or human services field (LSW, LCSW, LPC, LMFT) or licensed RN (BSN preferred)
• Minimum of three years of relevant clinical experience
• Experience with adult and child/adolescent populations in mental health and substance use disorder services preferred
• General knowledge of best practices in behavioral health, especially with special needs or priority populations
• Certification in substance use disorders and supervisory experience in behavioral health preferred
