Job Summary

To support quality healthcare services for individuals with serious mental and chronic health conditions, the full-time Pennsylvania Licensed Registered Nurse will coordinate health-related training, provide clinical expertise, and facilitate access to care while working remotely.

Key responsibilities:

• Coordinate and promote quality healthcare services for individuals with co-morbid conditions through training and technical assistance
• Develop and implement wellness plans and programs to enhance compliance with medication and medical appointments
• Advocate for clients by facilitating access to physical and behavioral health services and conducting health surveys

Required qualifications:

• Graduate from a Specialty Training Program in Nursing (Required)
• Bachelor's Degree in a related field (Preferred)
• Minimum of 3 years of nursing experience (Required)
• Basic Life Support Certification (Required)
• Licensed Registered Nurse in Pennsylvania (Required)
