Job Summary

A company is looking for a RN Care Navigator - PA (Remote).

Key Responsibilities

• Manage in and outbound calls to provide exceptional service to members
• Educate members on kidney health, dialysis, and transplant options while facilitating engagement in the Kidney Health Program
• Collaborate with healthcare teams to coordinate care and address patient needs, ensuring compliance with regulations and policies

Required Qualifications

• Active, unrestricted RN license required
• Bachelor's degree required
• CCM certification preferred
• Three years of experience in case management preferred
• Experience in a dialysis or transplant center preferred
