Job Summary

A company is looking for a RN Care Navigator (100% Remote in PA).

Key Responsibilities

• Manage a caseload of members, facilitating high-quality, cost-effective care
• Educate members on kidney health and coordinate care with healthcare providers
• Utilize community resources and support self-management for patients

Required Qualifications

• Active, unrestricted RN license
• Bachelor's degree
• CCM certification preferred
• Three years of experience in case management preferred
• Experience in a dialysis or transplant center preferred
