Pennsylvania Licensed RN Case Manager
Job Summary
To support a growing healthcare initiative, the full-time Pennsylvania Licensed RN Case Manager will conduct comprehensive member assessments, develop care plans, and coordinate services remotely from Monday to Friday, 8 am to 5 pm EST.
Key responsibilities
- Conduct comprehensive assessments of member healthcare needs and eligibility
- Develop and support care plans that promote optimal and cost-effective outcomes
- Coordinate services and advocate for members across healthcare programs and resources
Required qualifications
- Active unrestricted RN license in Pennsylvania or Compact RN License
- Minimum 3 years of clinical nursing experience
- At least 2 years of case management experience
- Experience in hospital, home health, ambulatory care, or similar settings
- High School Diploma or equivalent
