Job Summary

A company is looking for a CHC Assistant Service Coordinator for a remote position.

Key Responsibilities:
  • Establish and maintain relationships with participants and providers to facilitate service coordination
  • Assist participants in accessing long-term services and supports (LTSS) and ensure compliance with care plans
  • Collect and analyze program data to monitor participant progress and prepare reports for regulatory compliance
Required Qualifications:
  • High School Diploma required
  • 2 years of data entry or customer service experience required
  • Preferred experience working with individuals with disabilities or seniors in need of LTSS
  • Cultural competency and sensitivity to diverse backgrounds
  • Highly preferred: Nursing facility experience
