Job Summary
A company is looking for a CHC Assistant Service Coordinator for a remote position.
Key Responsibilities:
- Establish and maintain relationships with participants and providers to facilitate service coordination
- Assist participants in accessing long-term services and supports (LTSS) and ensure compliance with care plans
- Collect and analyze program data to monitor participant progress and prepare reports for regulatory compliance
Required Qualifications:
- High School Diploma required
- 2 years of data entry or customer service experience required
- Preferred experience working with individuals with disabilities or seniors in need of LTSS
- Cultural competency and sensitivity to diverse backgrounds
- Highly preferred: Nursing facility experience
