Job Summary

A company is looking for a Supervisor Service Coordinator.

Key Responsibilities

• Oversee day-to-day operations of the LTSS department, including service plan development and benefits coordination
• Serve as a subject matter expert for LTSS training for internal and external audiences
• Support coordination of LTSS with clinical and social services to enhance Participant care quality

Required Qualifications

• Bachelor's degree required
• Pennsylvania licensed RN, LSW, or LPC required
• 1 to 3 years of leadership and/or supervisory experience
• 3 years of experience in social service or healthcare-related field
• Experience working with people with disabilities or seniors in need of LTSS
