Job Summary

A company is looking for a Primary Care TeleCare Nurse.

Key Responsibilities

• Evaluate and assess healthcare needs of patients through telephonic evaluations
• Deliver, manage, and coordinate care within the Ambulatory Care Nursing domain
• Ensure continuity of care by communicating with the healthcare team

Required Qualifications, Training, and Education

• Minimum of 3 years recent clinical experience in critical care, emergency department, home health, or physician practice nursing
• At least 1 year of telephone triage experience is strongly preferred
• Associates Degree in Nursing from an accredited school required; BSN strongly preferred
• PA RN license or a multi-state license through the Nursing Licensure Compact
• Provider BLS certification required from the American Heart Association or American Red Cross
