Job Summary

A company is looking for a Primary Care TeleCare Nurse, Ambulatory.

Key Responsibilities

• Evaluate and assess the healthcare needs of patients through telephonic evaluation of reported signs and symptoms
• Deliver, manage, and coordinate care services within the domain of Ambulatory Care Nursing
• Ensure continuity of care through effective communication with other members of the patient's healthcare team

Required Qualifications, Training, and Education

• Minimum of 3 years recent clinical experience in critical care, emergency department, home health, or physician practice nursing
• At least 1 year of telephone triage experience is strongly preferred
• Associates Degree from an accredited School of Nursing is required
• BSN is strongly preferred
• PA RN license or a multi-state license verified through the Nursing Licensure Compact is required
