Job Summary

A company is looking for a Primary Care TeleCare Nurse, Ambulatory.

Key Responsibilities

• Evaluate and assess healthcare needs of patients through telephonic evaluations
• Deliver, manage, and coordinate care within the domain of Ambulatory Care Nursing
• Ensure continuity of care through communication with the healthcare team

Required Qualifications, Training, and Education

• Minimum of 3 years of recent clinical experience in critical care, emergency department, home health, or physician practice nursing
• At least 1 year of telephone triage experience is strongly preferred
• Associate's Degree in Nursing from an accredited school required; BSN strongly preferred
• PA RN license or a multi-state license through the Nursing Licensure Compact required
• Provider BLS certification from American Heart Association or American Red Cross required
