Pennsylvania Licensed Telehealth Physician

Job Summary

To support innovative care delivery for vulnerable populations, the part-time Pennsylvania Licensed Telehealth Physician will conduct comprehensive telehealth assessments, synthesize medical and social histories, and create actionable clinical plans in a fully remote environment.

Key responsibilities

• Conduct comprehensive telehealth initial visits for enrolled members (15-20 min)
• Synthesize complex medical and social histories and identify clinical priorities and risks
• Create concise, actionable clinical plans and handoffs for Community Health Workers and care navigators

Required qualifications

• MD or DO with active, unrestricted Pennsylvania license
• Board-certified or board-eligible in Family Medicine, Internal Medicine, Geriatrics, or related primary care specialty preferred
• Substantial experience with Medicare/Medicaid populations or complex chronic disease management
• Skilled at telehealth assessment and building rapport virtually
• Comfortable working in an iterative, startup-like pilot environment
