Pennsylvania Licensed Telehealth RN

Job Summary

A company is looking for a Telehealth RN to provide care management for geriatric behavioral health patients.

Key Responsibilities

• Conduct a minimum of 60 minutes of patient and caregiver interactions monthly through telehealth or phone
• Monitor mental health symptoms, cognitive changes, and medication adherence
• Support dementia care planning and coordinate services within a multidisciplinary team

Qualifications

• Active, unrestricted Pennsylvania RN license
• Experience in geriatric care, behavioral health, or dementia care preferred
• Understanding of mental health conditions in older adults
• Experience in supporting caregivers and families
• Comfortable delivering care via telehealth and phone-based outreach
