Job Summary

A company is looking for a Telephonic Care Manager in the Community HealthChoices department.

Key Responsibilities

• Coordinate care and provide health education to Health Plan members through telephonic collaboration
• Conduct comprehensive assessments and develop individualized care plans in collaboration with members and healthcare providers
• Engage members in preventive health services and manage medication adherence issues

Qualifications

• Minimum of 2 years of experience in a clinical setting and case management nursing required
• BSN preferred
• Ability to interact professionally with healthcare professionals required
• Computer proficiency required
• Registered Nurse (RN) licensure required
