Job Summary

To support the Special Needs Plan team, a full-time Pennsylvania Licensed Telephonic Care Manager will coordinate care and provide health education to members through telephonic collaboration, primarily working remotely during standard EST daylight hours.

Key responsibilities:
  • Conduct comprehensive assessments of members' medical, behavioral, and social needs, and develop individualized care plans
  • Engage with members to close gaps in preventive care and assist with scheduling necessary health services
  • Document all interactions and care management activities in the Health Plan's tracking system, identifying trends for improvement
Required qualifications:
  • Minimum of 2 years of experience in a clinical setting and case management nursing required; 5 years preferred
  • BSN preferred
  • Registered Nurse (RN) licensure in Pennsylvania or a participating NLC state required
  • Case management certification or approved clinical certification preferred
  • Ability to maintain a private, secure workspace in a home office setting
