Job Summary

Managing a caseload of participants, the full-time Pennsylvania Licensed Telephonic Care Manager will provide episodic care management and resources remotely, focusing on care coordination, health education, and developing comprehensive care plans to address participants' medical, behavioral, and social needs.

Key responsibilities:
  • Contact participants to assist with scheduling preventive health care services and conduct comprehensive assessments of their needs
  • Engage participants in education and self-management programs while developing individualized care plans in collaboration with primary care providers
  • Document all interactions in the care management tracking system and identify trends for improvement based on participant feedback
Required qualifications:
  • Minimum of 2 years of experience in a clinical setting and case management nursing
  • Experience with Service Coordination in community and nursing facility settings preferred
  • Strong understanding of Medicaid, Medicare, and LTSS waiver programs preferred
  • BSN preferred
  • Current Registered Nurse (RN) licensure required
