Pennsylvania Licensed Telephonic Care Manager

Job Summary

Providing telephonic case management for pediatric patients, the full-time Pennsylvania Licensed Telephonic Care Manager will coordinate care, develop comprehensive care plans, and engage with members and their caregivers remotely during daylight hours, Monday through Friday.

Key responsibilities:

• Conduct comprehensive assessments of members' medical, behavioral, and social needs while identifying gaps in preventive care
• Collaborate with interdisciplinary teams to update care plans and communicate recommendations to members and providers
• Engage members in education and self-management programs to promote healthy lifestyles and reduce unnecessary ER visits

Required qualifications:

• Minimum of 2 years of experience in a clinical setting and case management nursing required; BSN preferred
• Pediatric nursing experience is preferred
• Home health experience is a plus
• Current Registered Nurse (RN) licensure in Pennsylvania or a multistate license from the NLC
• Case management certification or approved clinical certification preferred
