Job Summary

Providing telephonic care in a part-time capacity, the Pennsylvania Licensed Telephonic Nurse Practitioner will manage patient care coordination, deliver quality telehealth services, and conduct comprehensive assessments remotely, requiring evening, night, and weekend availability.

Key responsibilities:
  • Coordinate and manage care for members through telephonic communication with caregivers, facilities, and providers
  • Deliver acute care and document findings in compliance with regulations while utilizing electronic medical records
  • Facilitate care transitions and monitor member needs, adjusting care plans as necessary
Required qualifications:
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
  • Active and unrestricted Nurse Practitioner license in Pennsylvania and ability to obtain in other required states
  • Board certified as a Nurse Practitioner through a national board
  • Current DEA licensure/prescriptive authority or ability to obtain post-hire
  • Proficient in documenting medical information using written and electronic medical records
