Job Summary

Providing telephonic care in a part-time capacity, the Pennsylvania Licensed Telephonic Nurse Practitioner will manage patient care coordination, deliver quality telehealth services, and collaborate with interdisciplinary teams while working remotely during evenings, nights, and rotating weekends.

Key responsibilities:
  • Coordinate and manage care for members through telephonic communication with caregivers, facilities, and providers
  • Deliver acute care and comprehensive assessments while documenting findings in compliance with regulations
  • Facilitate care transitions and monitor patient needs, adjusting care plans as necessary
Required qualifications:
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
  • Active and unrestricted Nurse Practitioner license in Pennsylvania
  • Board certification through a national board as a Nurse Practitioner
  • Current DEA licensure or prescriptive authority, or ability to obtain post-hire
  • Proficient in electronic medical records and documentation
