Job Summary

A company is looking for a Transition of Care Associate in Care Management ED Outreach.

Key Responsibilities

• Conducts patient outreach to facilitate post-discharge care coordination and engagement
• Manages post-discharge and care gap reports, ensuring accurate documentation and tracking
• Collaborates with interdisciplinary teams to enhance patient care activities and communication

Required Qualifications

• High School Diploma/GED or Specialized Diploma in Medical Assistant or related field
• 3 years of experience in a complex health system environment or less than 1 year with a Specialized Diploma
• Proficiency in Microsoft Office applications
• Knowledge of medical terminology and electronic medical records
• Ability to maintain strict confidentiality and adapt to changing priorities
