Job Summary

A company is looking for a Transition of Care Associate in Care Management ED Outreach.

Key Responsibilities

• Conduct post-discharge outreach and care coordination for patients in collaboration with the interdisciplinary team
• Manage tracking systems for care gap reports and ensure documentation is updated
• Organize patient care activities and facilitate communication among all parties involved in patient treatment

Required Qualifications, Training, and Education

• High School Diploma/GED or Specialized Diploma in Medical Assistant or related field
• 3 years of experience in a complex health system environment or less than 1 year with a Specialized Diploma
• Proficiency in Microsoft Office applications
• Knowledge of medical terminology and electronic medical records
• Ability to maintain strict confidentiality and adapt to changing priorities
