Pennsylvania RN Case Manager

Job Summary

A company is looking for an RN Case Manager to coordinate comprehensive patient care across various domains.

Key Responsibilities

• Conduct initial intake calls and ongoing patient outreach to review and update care plans
• Provide care coordination for a panel of complex patients and support during care transitions
• Collaborate with multidisciplinary teams to address care gaps and implement interventions

Required Qualifications

• Current RN License in good standing in the state of practice
• Bachelor's degree in nursing or an associate degree in nursing with another bachelor's degree
• Minimum of 4 years of clinical experience, including 2 years in case management
• Certified Case Manager (CCM) certification or commitment to obtain it when eligible
• Geriatric care experience is highly desired
