Pennsylvania RN Case Manager

Job Summary

Focused on coordinating comprehensive patient care, the full-time Pennsylvania RN Case Manager will conduct intake calls, provide ongoing care coordination for complex patients, and collaborate with a multidisciplinary team, all while working remotely.

Key responsibilities

• Conduct initial intake calls and scheduled patient outreach to review and update care plans
• Provide ongoing care coordination for an assigned panel of complex patients and support them during care transitions
• Identify and implement interventions to address gaps in care and collaborate with the care team and external providers

Required qualifications

• Current RN License in good standing in Pennsylvania
• Bachelor's degree in nursing or an associate's degree in nursing with another bachelor's degree
• Minimum of 4 years of clinical experience, including at least 2 years in case management
• Certified Case Manager (CCM) certification or commitment to obtain when eligible
• Geriatric care experience is highly desired
