Job Summary

A company is looking for a Pharmacist (West & Central U.S. Coverage).

Key Responsibilities

• Perform Medication Therapy Management (MTM) interventions and medication reconciliation
• Provide patient-centered education on medications and support chronic care initiatives
• Collaborate with healthcare providers on treatment planning and ensure accurate documentation

Required Qualifications

• Doctor of Pharmacy (PharmD)
• Active pharmacist license in at least one of the specified states
• Open to obtaining additional licensure as required
• Experience in MTM delivery and chronic disease management preferred
• Familiarity with programs such as CCM, TCM, RPM is beneficial
