Job Summary
To ensure optimal clinical care for patients with kidney disease, the remote Pharmacist - State Licensed will collaborate with an interdisciplinary team to manage medication regimens, engage in patient outreach, and drive clinical quality initiatives during standard working hours in the central time zone.
Key responsibilities
- Consult with patients and care teams to implement comprehensive pharmacy care management programs, including medication reconciliation and education
- Utilize Comprehensive Medication Management principles to optimize medication therapy regimens and ensure patient safety
- Drive medication-related clinical quality initiatives and participate in safety initiatives to minimize nephrotoxic medication use
Required qualifications
- Doctorate or bachelor's degree in pharmacy
- Unrestricted Pharmacy state license or ability to obtain one within 60 days of hire
- Strong comprehension of pharmaceutical therapies focused on patient quality of life
- Experience delivering educational seminars on pharmaceutical topics to clinicians and patients
- Intermediate skills with MS Office Suite, including Outlook and Teams
