Job Summary

A company is looking for a Pharmacy Care Coordinator.

Key Responsibilities

• Conduct outbound outreach to engage patients and introduce pharmacy care management services
• Schedule pharmacist consultations and coordinate continuity of care among care team members
• Serve as a liaison to resolve medication barriers and assist patients with enrollment in medication assistance programs

Required Qualifications

• High school diploma or equivalent
• Experience in healthcare, pharmacy, care coordination, or patient support roles
• Strong organizational skills and ability to manage multiple tasks
• Proficient in MS Office Suite, including Outlook and Teams
• Able to work effectively in a remote environment with a dedicated workspace
