Job Summary

A company is looking for a Pharmacy Care Coordinator.

Key Responsibilities

• Support Clinical Services staff with patient outreach, appointment scheduling, and communication with providers and pharmacies
• Enhance efficiency of patient interactions to maximize encounters and improve communication
• Coordinate care between patients and clinical staff to improve access to services and ensure positive patient experiences

Required Qualifications

• High School Diploma/GED (or higher)
• 6+ months of experience in healthcare operations (e.g., Clinic, Pharmacy, Scribe, PBM, or Health Plan)
