Pharmacy Care Navigator

Job Summary

Collaborating with interdisciplinary teams, the full-time Pharmacy Care Navigator will minimize treatment gaps and enhance medication adherence by providing medication reconciliation, access, and affordability services in a remote setting.

Key responsibilities:

• Act as the pharmacy team's first line of triage for medication access, adherence, and affordability needs, including managing prior authorizations and copay assistance
• Schedule and facilitate initial pharmacist consult visits, ensuring accurate medication history and communication with external providers
• Proactively screen disease state registries and medication adherence reports to confirm member eligibility for clinical pharmacy programs

Required qualifications:

• 6 years of experience as a pharmacy technician in a retail and/or outpatient pharmacy
• Strong knowledge of medication access and adherence tools and resources
• Experience with medication reconciliation and communication with healthcare providers
• Ability to manage multiple tasks and prioritize effectively in a remote environment
• Familiarity with clinical pharmacy programs and prior authorization processes
