Job Summary

A company is looking for a Clinical Pharmacist (MTM Support).

Key Responsibilities

Evaluates medication profiles to optimize drug therapy and minimize adverse reactions
Recommends modifications to medications, including alternatives and discontinuations
Conducts telephonic comprehensive medication reviews with members and collaborates with physicians

Required Qualifications

Bachelor's or Doctorate of Pharmacy degree
Minimum one year of pharmacy residency training or equivalent experience recommended
1-2 years of experience in a pharmacy or related healthcare environment
Current, valid, and unrestricted state Pharmacy license required
May need additional Pharmacy licensure in multiple states within six months of employment
