Job Summary
To enhance patient care and support medication adherence, the part-time Pharmacy Quality Coordinator will make outreach calls, identify root causes of adherence issues, and provide administrative support for performance programs while working remotely.
Key responsibilities
- Make outreach calls to members to address care gap issues and provide education on medication adherence
- Maintain internal trackers and create reports to monitor patient progress based on CMS Star metrics
- Communicate with physician offices to clarify prescription orders and follow up on pharmacist recommendations
Required qualifications
- High School Diploma or GED
- Working knowledge of MS Word and Excel
- Ability to maintain member confidentiality in compliance with HIPAA requirements
- Strong organizational skills with the ability to prioritize daily tasks
- Excellent verbal and written communication skills
