A healthcare organization is seeking a Population Health Care Coordinator.

Key Responsibilities:

• Conduct outreach to patients in need of preventative care and chronic disease review visits
• Engage with new and existing patients to ensure appropriate care and scheduling
• Identify and address barriers to care, collaborating with local teams for patient-centered solutions

Required Qualifications:

• 2-5 years' experience in a clinical setting with front desk or Medical Assistant duties
• Knowledge of Epic: Cadence or Epicare
• Ability to work independently and prioritize tasks effectively
• CMA certification and Bachelor's degree preferred
• 2 years' experience in an outpatient setting with insurance training
