A company is looking for a Population Health Care Coordinator.

Key Responsibilities:

• Conduct preventative care outreach to patients in need
• Close care gaps for chronic diseases through proactive outreach
• Engage and schedule new patients for care, addressing social determinant barriers

Required Qualifications:

• 2-5 years' clinical experience with front desk or Medical Assistant focus
• Knowledge of Epic: Cadence or Epicare or both
• Ability to work under pressure and prioritize effectively
• Ability to work independently with limited supervision
• CMA certification and Bachelor's degree preferred
