Population Health Coordinator

Job Summary

A company is looking for a Population Health Coordinator to support high-quality care delivery to specific member populations.

Key Responsibilities

• Manage data and registries for special populations, facilitating care coordination and quality improvement initiatives
• Provide direct member care through outreach, assessments, and education on health and recovery options
• Support pilot projects and administrative tasks as directed by leadership, contributing across medical, mental health, and social care areas

Required Qualifications

• Registered Nurse (RN) or Licensed Practical Nurse (LPN) for Medical Focus; Licensed Clinical Social Worker for Mental Health Focus; or relevant certification for Social Care Focus
• At least 5 years of experience in direct patient care or supervision in relevant fields
• Strong understanding of care coordination, case management, and healthcare regulations
• Proficiency with electronic medical records and documentation tools
• Ability to work flexibly, including adjusting hours to meet member needs
