Job Summary

Supporting members with low medical complexity, the full-time Population Health Coordinator will navigate health and social care resources, address health-related social needs, and ensure quality gap closure in a remote environment.

Key responsibilities

• Coordinates clinical and social assistance services for members based on queue prompts
• Utilizes member benefits platforms to determine eligibility and facilitate referrals to appropriate care resources
• Documents interactions and tracks tasks using care facilitation, EHR, and scheduling platforms

Required qualifications

• 2+ years of relevant experience in a healthcare or social services environment
• High school degree or equivalent
• Proficiency in using technology platforms for data management and communication
• Experience in customer service and member engagement
• Strong organizational and triage skills
