Population Health Manager

Job Summary

A company is looking for a Population Health Manager in Chicago, IL.

Key Responsibilities:

• Improve Accountable Care Team (Provider) engagement and awareness of quality and service offerings
• Coach on pre-visit planning, gap closure in care, and high-risk patient follow-up
• Develop initiatives to improve performance against value-based contract metrics and establish strong relationships with Accountable Care Teams

Qualifications:

• Bachelor's degree or equivalent
• 3+ years of client-facing experience in healthcare or relevant knowledge and skills
• Experience working with physician practices and knowledge of Medicare advantage, managed care regulations, and ACO strategy implementation
• Strategic and entrepreneurial mindset with strong collaboration and communication skills
• Demonstrated understanding of healthcare metrics and analytical skills
