Population Health Manager

Job Summary

A company is looking for a Manager of Population Health.

Key Responsibilities:

• Lead project management to ensure progress on Sexual and Reproductive Health, Pregnancy, and Primary Care Network Program development
• Drive quality improvement initiatives and rapid improvement cycles focused on SRH, pregnancy, and primary care
• Support execution of population health evaluation deliverables using an implementation science framework

Required Qualifications:

• Bachelor's degree
• 5+ years of professional experience, with 2+ years in care strategy and related implementation science or quality improvement
• Knowledge of the healthcare industry and value-based payment
• Strong project management skills
• Experience leveraging claims or operational data to inform decision-making
