Job Summary

A company is looking for a Population Health Patient Navigator.

Key Responsibilities

• Establishes relationships with patients and serves as their primary point of contact
• Collects and analyzes market data to assist in finding payor populations and verifies insurance benefits for obesity treatment
• Removes barriers to care by identifying resources and coordinating appointments for patients

Required Qualifications

• Bachelor's degree in Healthcare Management, Public Health, or related field, or equivalent experience
• Two years of experience in a related field with an Associate's degree, or four years with a high school diploma
• Preferred: Bachelor's degree in Healthcare Management or Public Health
• Three years of experience in the healthcare field, including one year in statistical data analysis
