Job Summary

A company is looking for a Clinical Guide, Population Health RN.

Key Responsibilities

• Perform clinical triage and assess member health concerns using evidence-based protocols
• Conduct proactive outreach to engage high-risk members and assist with care decisions
• Coordinate care, including scheduling appointments and addressing barriers to preventative care

Required Qualifications

• RN, ADN, or BSN degree
• Active RN license in a compact state
• 5+ years of direct patient care experience, preferably in family medicine
• Experience in population health or disease management
• Care management or chronic condition support experience desired
