Job Summary

Engaging patients and providers through a data-driven approach, the full-time Hybrid Population Health Specialist will implement and optimize value-based care workflows while supporting population health management initiatives remotely in Bedford, NH.

Key responsibilities

• Support the Population Health Manager in implementing provider engagement initiatives at practice or clinic levels
• Contribute to improving provider experiences with value-based care through practice-based support and coaching
• Coordinate patient health promotion efforts and manage care gaps to enhance population health outcomes

Required qualifications

• Bachelor's degree in Healthcare Administration, Business Administration, or a related field
• 1+ years of healthcare experience in roles such as health coach, medical assistant, or patient navigation
• Ability to thrive in a fast-paced and dynamic environment
• Effective communication skills for interaction with multi-disciplinary teams and patients
• Strong organizational skills with a focus on meeting timelines and goals
